Your Referral Journey

Implementing Our Care Plan For You

There are many ways you may have arrived here. Sometimes your social worker or family member may have been in touch, or you’ve reached out on your own. No matter how you’ve arrived here, we’re glad to meet you.

You may be referred to us by a social worker who has assessed your needs and perhaps obtained funding for your care from your local council depending on their budgeting costs. The social worker will either independently or jointly do a home visit with a director or care manager from All New Beginnings Ltd. They will carry out an individualised holistic needs and risk assessment which will be compiled into a care plan according to the needs identified and in line with the available budget. The budget from your local council will be discussed through either the independent living fund (ILF) or self direct support (SDS), along with agreeing a start date which will form a written agreement. Creation of a care plan will be put into place prior to the support workers being introduced to the service users.

Alternatively, you may refer yourself to us, or be referred to us from a carer, family member or a friend. In any of these cases, we will hold a telephone consultation, organise your needs via email, in person, or with a social worker recommendation.
