Implementing Our Care Plan for You
​There are many ways you may have arrived here. Sometimes your social worker or family member may have been in touch, or you’ve reached out on your own. No matter how you’ve arrived here, we’re glad to meet you.

You may be referred to us by a social worker who has assessed your needs and perhaps obtained funding for your care from your local council depending on their budgeting costs. The social worker will either independently or jointly do a home visit with a director or care manager from All New Beginnings. They will carry out an individualised holistic needs and risk assessment which will be compiled into a care plan according to the needs identified and in line with the available budget. The budget from your local council will be discussed through either the independent living fund (ILF) or self-direct support (SDS). Along with agreeing to a start date which will form a written agreement. Creation of a care plan will be put into place before the support workers are introduced to the service users.

Self Referrals
Alternatively, you may refer yourself to us, or be referred to us by a carer, family member or a friend. In any of these cases, we will hold a telephone consultation, and organise your needs via email, in person, or with a social worker recommendation.
A home visit will be carried out by one of our experienced care service team members from All New Beginnings, and they will carry out an individualised, comprehensive needs and risk assessment which will be compiled into a care plan according to the requirements identified, and in line with the available budget. A private budget will be discussed along with agreeing on a start date which will form a written agreement. Creation of a care plan will be put into place before support workers are introduced to service users.
